Telehealth Privacy Notice
Effective date: October 14, 2026
This notice explains how Pure for Men, LLC ("Pure for Men," "we," "us") collects, uses, shares and protects personal information, including health information, in connection with Pure for Men Telehealth. It also serves as our Consumer Health Data Privacy Policy. It adds to our Privacy Policy. Where the two differ for telehealth or health information, this notice applies.
1. Who provides your care
Pure for Men offers the telehealth program, markets it and provides customer support. Pure for Men does not provide medical care, review your medical history or make prescribing decisions.
- FUSE Health operates the telehealth platform where you complete your intake, visits and checkout.
- Beluga Health provides clinical care through independent, state-licensed healthcare providers. Your provider decides whether treatment is appropriate.
- The Pharmacy Hub, a licensed 503A pharmacy, fills and ships medication if it is prescribed.
Your intake answers, medical history, visit records and prescriptions are collected and kept by these organizations under their own privacy practices, including the federal HIPAA Privacy Rule where it applies to them. You can request their notices of privacy practices from them directly, or ask our telehealth support team and we will send them to you.
2. Information Pure for Men receives
- Information you give us. When you contact our support team, we receive your name, contact details and whatever you choose to tell us.
- Program information from the platform. To run the program and support you, we may receive your name, contact details, the program you enrolled in, and the status of your order, subscription, shipment and payment.
- Website information. When you visit our telehealth pages, we receive standard technical information such as your IP address, browser type and pages viewed.
We do not receive your intake questionnaire answers, medical history, provider notes or prescription records from the platform. If you choose to share health details with our support team, we use them only to help with your request.
3. How we use it
- To operate the program, answer your questions and resolve order, billing and shipping issues.
- To send you service messages about your order, subscription or account.
- To meet legal, regulatory and payment-processor requirements, and to prevent fraud.
- To understand overall site traffic using aggregated, non-identifying information.
We do not sell personal information or health information. We do not use information about your telehealth visit, program or orders to target advertising to you, and we do not use it to market other products to you without your consent.
4. Cookies and tracking on telehealth pages
Our telehealth pages currently use the same cookies and similar technologies as the rest of our website. These include analytics tools and advertising tools, such as those from Google, Meta and Snapchat, which may record that you viewed these pages. We are removing advertising tools from our telehealth pages. You provide your medical information in the intake on the FUSE Health platform, which is covered by FUSE Health's privacy practices. You can opt out of the sale or sharing of your information for targeted advertising on our Do not sell my information page and through your browser's privacy settings.
5. When we share information

- As part of a merger, sale or transfer of our business, subject to this notice.
We share consumer health data only with your consent or as needed to provide the service you requested.
6. How we protect it
We limit access to telehealth-related information to team members who need it to support you, use encrypted connections, and require our service providers to protect the information they handle for us. No system is completely secure. If a breach affects your information, we will notify you as required by law.
7. How long we keep it
We keep telehealth-related information only as long as needed to run the program, support you, and meet legal, tax and accounting requirements, and then delete or de-identify it.
8. Your rights
Depending on where you live, you may have the right to:
